This is the booklet we distribute in South Africa. It contains most of the information on this site in an easy to read A-Z format. It also has other information of relevance to South Africans, so you will have to edit it a bit to adapt it to your country.It is in Adobe Acrobat format. It was designed to be laid out in A6 format (1/4 of an A4). This is handy since it fits into a shirt or back pocket. Americans - hey the only thing we know about Letter or Legal page sizes is when our Laser Printer keeps asking for one:)
What you do is print it out, chop it up with a guillotine, and lay it out on strips of blank A4 cut in half, width ways. You start with the first page and the last page together, then the second and the second last, and so forth. You have to get the numbers right so the final page is the middle one: the numbers will be consecutive. Sounds complicated ? Have a go and you will get the idea. Then take these to a copy shop, who can then produce the booklet, or of course do it yourselves with a photocopier.

This is the Survey Form used by the South African group. It starts off with a list of drugs that are around locally, and has columns to get a profile of periodicity and dosage. There is also a check list of side effects, which were compiled from the most common ones specified in previous surveys. On the back, there is quite a bit of room for feedback for the organisation, to help gauge how effective we are. Lastly there is a checklists for the quality of the party where you are doing the survey, which you can pass back on to the organisers. It's laid out on A4 paper size and intended to be a double sided survey.
We intend to create an on-line form of this soon, and encourage you to stick to this standard format as much as possible, so that you will be able to enter this data into a global database for the benefit of all.
